New Zealand — my fix was simply enabling hardware decode in TiviMate. Had it set to auto which was sometimes using software decode on higher bitrate streams.
 
Hardware decode should always be enabled explicitly on Android TV devices. Auto can sometimes choose software decode incorrectly.
 
Canada — asked support for a closer server region after trying all local fixes. They moved me to a server with lower latency to my location. Big improvement.
 
CDN region assignment is worth asking about if local fixes do not fully resolve the issue. It is something providers can often adjust on their end.
 
Summary: Ethernet, QoS, hardware decode, DNS, correct server region — five settings that collectively resolve most buffering issues. Save this thread.
